If you are referred to another healthcare professional by Dr. Evan, you agree that any information related to your health may be shared between Dr. Evan and that/those professional/s.
CLIENTS ARE RESPONSIBLE FOR KEEPING ALL APPOINTMENTS ON TIME, or giving a minimum of forty-eight hours' notice of change. Monday appointments need to be cancelled/changed by 9:00 AM on the previous Friday via phone call (not email). Cancelled or missed appointments with inadequate notice given require payment in full before another appointment is given. Late arrivals may not receive a full session, but will be charged for them. EXCEPTION: Acts of God. If your school district and/or mine have determined that the weather is not conducive to safe travel for children to school that day, you will not incur a missed appointment fee.
Clients are asked to re-order remedies and/or supplements at least one week in advance; occasionally vendors are not able to ship the needed supplements quickly.
Dr. Evan dialogs personally with clients between 9:00 AM and 10:00 AM ET / EDST, Mondays through Fridays at 610-933-1982. Dr. Evans' testing schedule does not permit return phone calls at other times. Voice-mail is provided to place orders and to give timely notice of appointment changes.
